Community-Based Services

Community-based services involve a combination of case management and counseling for people diagnosed with a serious mental illness who have experienced hospitalizations related to their mental illness. 

Our staff provides assistance and education to individuals to develop their life skills and establish supports they need to live successfully in the community. A wide variety of life situations can contribute to stressors that trigger experienced symptoms, so our staff is prepared to provide assistance, education, and training in almost all areas of life that may be contributing to ongoing life struggles. Our goal is to help our clients develop the support systems and skills they need to achieve their personal life goals.

We provide services in a variety of community locations based on the client’s needs. Education and support take place in homes, day programs, and other locations our clients frequent. Our flexible approach empowers our clients to overcome barriers that prevent them from reaching their life goals.

We have agreements with psychiatrists to enable us to get our new clients, who need an outpatient psychiatrist, an appointment within 7 days of discharge from the hospital, instead of having to wait 3-6 months for a follow-up appointment.
