The YES Waiver program offers home and community-based services and support to families of children or adolescents with severe emotional and/or behavioral needs. Through a family-driven, team-based approach, the YES program seeks to meet the unique needs of a child and their family by offering individualized services that focus on empowering the family’s strengths.

Expanded Medicaid Services:

• community living supports
• counseling
• family support service
• case management
• professional services and support services for children with intensive needs to prevent out-of-home placements and reduce unnecessary hospitalizations

For youth, ages 3-18.
