The Charlie Naylor Campus offers youth and young adults:

Our disability care services include:

  • Pressure Relief: Mobility issues can lead to pressure sores, debecutis ulcers and, in severe cases, the need for wound care. BrightStar Care caregivers are trained to provide pressure relief every 15 minutes for wheelchair-bound patients, and to turn bed-bound patients every two hours to prevent complications caused by prolonged immobility. Our CNAs are also trained in the transfer of patients using Hoyer lifts and slide boards, and use the proper techniques to help patients avoid injuries and falls during transport.
  • Care for Special Needs Children: Babies, infants and children living with disabilities or special needs have unique care requirements. BrightStar Care trained professionals specialize in administering in-home pediatric care and can provide comprehensive case management and support for children with special needs.
  • Special Event and Respite Care: Whether you need assistance caring for your loved one during a wedding or holiday celebration, BrightStar Care care professionals can provide help on a short-term basis so you and your family can make time for a vacation or receive additional support during a special event.
  • Travel Companionship: Travel and transportation can be challenging for individuals with disabilities and special needs. BrightStar Care provides travel companion care to help make travel accessible for people with special needs, enabling them to experience more in life.
  • Medical Procedure Transportation: BrightStar Care care professionals are available to help those who need assistance with a scheduled medical procedure or doctor’s appointment. Your compassionate care professional will:
  • Pick you up at home and taking you to the hospital or doctor’s office.
  • Wait until the procedure is completed and signing you out.
  • Safely transport you home.
  • If needed, stop at a pharmacy, grocery store, or restaurant if you need prescription, medical supplies, groceries, or food.
  • Make sure you are safely settled in your home after your procedure.

Additional options:

  • We can stay with you in your home after the procedure and assist with meal preparation, bathing, toileting, laundry, light cleaning, and medication reminders.
  • A Registered Nurse (RN) can visit after you are home and change your dressings, perform wound care or infection monitoring.
  • An RN patient advocate can help coordinate care, treatment, and communicate between you, your family and healthcare providers. We can attend appointments with you and help you understand options and make the best possible health care decision.

Journey is a respite program offering a weekly meeting to those with early to mid-stage dementia and their caregivers.

Trained volunteers will provide support and encouragement to participants in a community-based setting.

Meetings combine socialization, physical movement, art, music, and other cognitive-stimulating activities. Lunch will be served in a safe and comfortable environment.

While those with dementia and their caregivers navigate this challenging journey, our respite program is here to offer safe refuge, allowing caregivers a four-hour window to attend to their personal needs with the assurance that their loved one is being well cared for.

A Long-Term Care Ombudsman means "being an advocate for residents."  A Long-Term Care (LTC) Ombudsman listens to and addresses the concerns of nursing home and assisted living residents.  At all times, residents decide on the LTC Ombudsman's level of involvement. They can be counted on to respect residents' privacy and confidentiality. Some of the ways in which LTC Ombudsman serve residents include telling residents about their rights and empowering and supporting residents and their families to discuss concerns with staff.   An LTC Ombudsman completes State-supported training and is certified by the State Long-Term Care Ombudsman.

What is Physical Therapy?

Physical Therapy (PT) is an intervention that focuses on helping individuals (in TLC’s case, children) with physical impairments that are impacting their mobility, balance, coordination, quality of movement, or age-appropriate gross motor skills.

Pediatric physical therapists examine and treat children from birth to age 18 for symptoms related to genetic, neurological, and orthopedic conditions. At TLC Kids Therapy, a typical session may include manual techniques to address the limitations of myofascial or soft tissue mobility, positional techniques to facilitate postural alignment, and development of gross motor skills such as rolling, crawling, walking, jumping, riding a bike, climbing stairs, climbing stairs, and transitions between positions.

Other common interventions include flexibility training to maximize range of motion, general strength training through everyday play,  and gait training to improve running and walking patterns. Family education is essential to success in physical therapy and a recommended individualized home exercise program will be given to each family to ensure the carry-over of skilled physical therapy intervention.

Physical therapy commonly collaborates with occupation and speech therapy (if needed) to optimize your child’s success in the home and school environment.

What is Speech Therapy?

Speech Therapy (ST) is an intervention that focuses on improving a child’s speech and abilities to understand and express language.

Therapy includes coordinating the mouth to produce sounds to form words, increasing a child’s ability to understand and express language via different communication modalities (e.g., written, pictorial, body, signs, and the use of language through alternative communication systems such as computers, and iPads), and overall help strengthen and coordinate the muscles in the mouth for speech and feeding purposes.

In the latter, ST also focuses on assessing a child’s chewing skills, and whether they are moving their tongue correctly to thoroughly break down the food and propel it back for the swallow. Lastly, speech therapy can also help babies improve their ability to suck from a bottle and coordinate breathing while eating.

Hospice focuses on caring, not curing and in most cases care is provided in the patient's home. Hospice care also is provided in freestanding hospice centers, hospitals, nursing homes, and other long-term care facilities.

The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.

Among its major responsibilities, the interdisciplinary hospice team:

  • Manages the patient’s pain and symptoms; 
  • Assists the patient with the emotional psychosocial and spiritual aspects of dying;

Learners come in all forms, preschool kids, school-aged kids, college students, adult students, and everyone in between. Learning applies to not only the school but also to the developmental tasks needed to flourish in our lives.

AIM Knows that not all learners can get support from a public school system, where the majority of this evaluation/therapy happens. We also know as many families turn to medical providers, there is a difference between the way learning issues are identified and treated in the medical world and the educational world. They speak different languages. Learners and their families are often caught in the middle. We built Spark Assessment Center for learners who couldn’t or wouldn’t use a school for testing and diagnosis. My, how times have changed. Now, a typical Spark customer visits the office for:

  • Learning evaluations for private school students
  • Autism evaluation
  • ADHD evaluations
  • Testing for communication disorders
  • Updated testing for accommodations in college
  • Testing for accommodations on things like SAT, MCAT, LSAT, etc
  • IEEs
  • Complex, overlapping issues or differential diagnosis - Is the issue - dyslexia or AD/HD? Is this depression or a learning disability?
  • Further exploration of a medical diagnosis

Services for students with disabilities are available on every campus. Except for a small population, most students with disabilities attend their home campus. A wide variety of services and levels of support are provided on each campus. Supports may be provided within the general education classroom by a co-teacher, a paraprofessional, or a tutor.

Modified curriculum materials are provided when necessary, and students are given specialized instruction outside of the general education classroom when needed. Each student truly has an “individualized education plan” tailored just for them

Home and Community-Based Services (HCBS) is a Medicaid waiver program that provides services and support to individuals with intellectual disability disorders (IDDs), so they can continue to live with family or independently.

These services can be provided in your own home, your family's home, a foster care home where you live with another family, or a home in the community where the individual lives with two or three other people.

Everybody who is enrolled in the HCBS program will have a Service Coordinator who:

  • coordinates the development and implementation of the person's Individual Plan of Care (IPC)
  • coordinates various aspects of services delivered under the HCS Program and through other sources
  • keeps track of the person's progress or lack of progress.
