The Grace House setting and training allow women to overcome the addictive and destructive lifestyles that have kept them from becoming productive citizens.

Applicants must submit an application and be interviewed and evaluated to determine their desire and potential for participating in the Grace House program.

The Home and Community-based Services (HCS) program provides individualized services and supports to people with intellectual disabilities who are living with their families, in their own homes or in other community settings, such as small group homes where no more than four people live. The SLEW Cancer Wellness Center provides survivor and aftercare services that improve the quality of life for uninsured, underinsured, and low-income women recovering from and undergoing surgery, chemotherapy, and/or radiation treatment for cancer. Services are provided holistically at a single location so those who are unable to go from agency to agency for services due to health and mobility issues, budget constraints, and transportation limitations can access the care they need.
SLEW was initially modeled after an evidence-based program at the Mayo Clinic. The model addresses mind, body, and spirit, and has been proven to increase a patient’s perceived quality of life.

GRASP offers daily activities and programs for elderly persons to promote socialization, independence and better nutrition to live a longer, healthier and happier lifestyle. Individuals qualify for the program who are ambulatory and over age 60. Spouses and caregivers are also eligible to participate.
CONGREGATE MEALS
Hot noon meals are served daily except weekends at the GRASP Community Center in Converse. Professionally prepared meals meet one-third of the daily requirements for nutrition. All participants must register.
TRANSPORTATION
GRASP offers accessible daily bus transportation services for local seniors and disabled persons or living with disabilities.
ACTIVITIES
Individuals can participate in daily activities that include low-impact exercises, guest speakers, arts, crafts, games, classes, day trips, evening dinners, dances and other activities planned by the enrolled participants.

At the North Central Federal Clinic, we strive to provide high quality, customer-friendly health care services to Veterans as well as TRICARE beneficiaries.
PRIMARY CARE: VA patients are assigned a Patient Aligned Care Team health care. TRICARE patients are assigned to Family Medical Home health care team. These health care teams work collaboratively with other clinic staff to insure the best care available. Specialists are available by referral.
