The Gap program provides services to grandparents and other relatives who care for children in the absence of parents.

We support families by providing in-home assessments, case management services, and support groups for caregivers.

The Gap program builds on the family’s strengths and works with all family members to find solutions to the challenges of raising children in today’s complex world.

The Gap program offers the following services to support your family through our MCH Family Outreach offices:

Case Management
We assess the strengths and needs of your situation and a case manager meets with your family regularly to provide you with services geared toward strengthening your family.

Information and Referral
We help you access community resources and programs to assist you with everything from counseling and parent training to food assistance and childcare services.

Support Groups
Caregiver Empowerment Groups offer emotional support and advice on ways to deal with common problems many grandparents and caregivers face when raising children.

Assistance with Legal Issues
We help you find community programs and government agencies that provide legal assistance.

Workshops
We partner with other like-minded community organizations that offer workshops on topics important to you as a generational caregiver.

Services are designed for families with at least one child residing in the home between the ages of birth to 14 years. Eligible families should also live within a 45-mile radius of an MCH Outreach office.

All MCH Family Solutions services are confidential and free to eligible families.
