Our psychiatric service program gives us the ability to bring our expertise on-site to your residents. Our team of psychiatrists, advanced nurse practitioners, and physician assistants work hard to ensure your residents are on the least amount of medication that is required to effectively manage their needs.

Initial Assessment

A comprehensive psychiatric evaluation to determine the proper CMS-approved diagnosis and review appropriateness of all medications.

Medication Management

Consistent resident visits focusing on improving emotional and behavioral functioning while utilizing the minimum amount of medications necessary to manage symptoms and conditions.

Quarterly Treatment Plan Reviews

Collaborating with your interdisciplinary team reviewing pharmacy reports and treatment plans to review medications, behavioral issues, and provide Gradual Dose Reductions (GDR) where appropriate.

Pharmacy Reviews

Address any pharmacy recommendations for medication adjustments, polypharmacy, duplication or reductions of medications, while providing survey-driven documentation.

Gradual Dose Reduction Tracking

All reductions, whether successful or failed, are tracked and reported to avoid survey citations and F-Tag’s.

  • Currently on Antipsychotic, Antidepressant, Antianxiety, Mood Stabilizers, Stimulants, Hypnotic or any psychotropic medication

Proactive and consistent psychological services are provided to residents on-site in long-term care communities. Our dedicated team of doctoral-level licensed psychologists and licensed clinical social workers are experts in geriatric behavioral health.

Individual, Group and Family Psychotherapy

Because many facilities operate in rural areas where behavioral health provider shortages exist, and distance stands in the way of consistent care, we overcome these challenges through our Tele-Behavioral health program that mirrors our on-site programs.

Each provider is hand selected and trained to provide tele-behavioral health services. Providers interact with the residents through live, high definition audio and video equipment that comes at no cost to your facility. With telemedicine, research has found satisfaction to be high among residents. Tele-behavioral health is equivalent to on-site care in diagnostic accuracy, treatment effectiveness, quality of care and patient and family satisfaction.

The San Antonio AIDS Foundation provides a continuum of care to low-income HIV-positive men, women and transgender individuals. Our services include resource and benefits assistance, case management, mental health counseling, medical care, pharmacy services, housing, and a year-round hot meal program. We offer onsite and mobile testing for HIV and STDs, as well as HIV prevention education presentations in local public schools, at colleges and universities, adult probation facilities and teen detention programs.

Sisters Care In-Home Care provides caregivers in an adult's residence to assist them with bathing, dressing, grooming, meal preparation, organization, and companionship.

Services offered are paid through private pay, long-term care insurance, and A.A.C.O.G. funding.

Additionally, Sisters Care In-Home Care has a non-profit program called Project Elder Help that offers caregivers on a reduced fee basis if approved based on application and grant funds available.

Community Living Services – Intermediate Care Facility – Group Home

ResCare’s Group Home setting offers individuals with Intellectual and Developmental Disabilities (IDD) a more personal, home-like atmosphere.

Each resident receives their own bedroom with no more than three to four individuals in a house. Each home is staffed 24/7 and helps make the home feel more like a family setting. Meals are served in common areas, providing residents with an opportunity for social interaction and camaraderie.

Individuals in the group home share responsibilities with cooking, cleaning, shopping and other tasks related to home maintenance. The Group Home setting allows for an individual to live their life to the best of their abilities while being cared for and assisted as needed.

Mission Road Developmental Center provides residential care for adults with intellectual and other developmental disabilities through the ICF (Intermediate Care Facility) program and the HCS (Home and Community-Based Services) programs. The Texas Health & Human Services Commission monitor and license the HCS and the ICF group homes and the assisted living program. We have 12 HCS and 4 ICF for a total of 84 residents.

Home
Mission Road has 16 adult community homes throughout the city of San Antonio and one assisted living home on the main Mission Road campus for adults. Residents living in the homes are assisted by well-trained, 24-hour AWAKE staff. During the week, residents spend their day working, training for a job, volunteering or engaging in a social/habilitation program. Evenings are spent acquiring and refining the skills needed to live a more independent and fulfilled life. Weekends are for shopping, completing chores, recreational activities and spending time with friends.

Apartments
Mission Road Ministries sponsors a supervised apartment program for adults with intellectual and other developmental disabilities who need minimal supports. The goal of the program is to provide supportive services to optimize the independence and productivity of the adults in an affordable living arrangement.

Mission Road has three apartment complexes in San Antonio that provide affordable housing and services for the tenants. Suzanne Smith Management Company supervises the three apartments: Independence Square, Murray Manor and Meadow Brook. The apartments are Housing and Urban Development (HUD) subsidized apartment complexes designed exclusively for adults with intellectual and other developmental disabilities. The apartments have one or two bedroom options with a community room for gatherings and a laundry room.

Our state-of-the-art facility was designed with children in mind, allowing them to play, dream, and grow — all while meeting their physiological, developmental, nutrition, and social needs. Our goal, through PPEC, is to impact the lives of our medically complex children from the early stages of life all the way into early adulthood.

What is PPEC?
Prescribed pediatric extended care, or PPEC for short, is provided at a non-residential center that serves medically complex or technologically dependent children under the age of 21 who require short, long-term, or intermittent medical care due to medically complex conditions.

The ACP PPEC provides services to medically complex children who cannot attend a regular daycare. Our services meet the child’s physiological, developmental, nutrition, and social needs. We provide nursing and psychosocial services, developmental therapies, caregiver training, and personal care support to our children.
