We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs. Seniors 2000 adult day care program is designed to meet the needs of adults with functional impairments through individual plans of care by providing health, social, and related support services in a safe environment. This may include light exercises, individual floor dances,a pool table, table games, bingo, arts and crafts, and occasional "Zumba". Our centers have an open space setting for wheelchair bound individuals to move around. We provide a clean, safe, and comfortable environment for our clients to spend the day. Services include: • Transportation. We arrange for our clients to have safe transportation to and from their homes daily, making adult care services convenient. We hold Field Trips once in a while, enjoy visiting local libraries, local shops and or parks with our staff members accompanying a small group of clients. We keep our daily activities fresh everyday. Boredom is a thing of the past. • Therapeutic Activities. Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our adult day care can assist you with daily tasks such as mobility, eating, exercising, and grooming. • Healthy Meals. Our centers serve breakfast, lunch and a mid-afternoon snack, all prepared in our own central kitchen. A registered dietitian designs our menus, ensuring that all dietary needs are met.
This program is for adults (18+) with intellectual and development disabilities (IDD).
We contract with Amerigroup Health Care, Molina Health Care, Superior Health Plan, AACOG, UnitedStates Veterans ( VA ). If a client is eligible under the Medicaid Program, the services will be paid in full by Medicaid and will be at no cost to the client.

We are continuing to provide diapers as often as once per week during April. Limited formula and baby food, but we will provide as we have available. Diaper Days distribution on Weds. 9:00am - 11:00am and 1:00pm-3:00pm. Protocols are in place for health: after sign-in, each participant will be screened, materials will be delivered curbside. By appointment is preferable.

Assists seniors with diapers, wipes, bed pads, and toiletries every month. We saw a huge need for these items in the community and knew we were called to help. If you are a senior 65+, and have a medical condition that keeps you out of the workforce, then we can help you.

As of Wednesday, April 1, 2020, our campus and community homes will remain closed to visitors through Monday, May 4. This also includes the continued closure of our Free To Be Day Activity Program at our southside campus and all programs at Unicorn Centers. This is subject to change based on evolving recommendations from health officials. Residents may communicate through video chat, email, and phone calls Residents who leave for a home visit are required to stay for 14 days and must be without symptoms upon their return. Program / case managers are available for information and assistance.

Mission Road Developmental Center provides residential care for adults with intellectual and other developmental disabilities through the ICF (Intermediate Care Facility) program and the HCS (Home and Community-Based Services) programs. The Texas Health & Human Services Commission monitor and license the HCS and the ICF group homes and the assisted living program. We have 12 HCS and 4 ICF for a total of 84 residents. Home Mission Road has 16 adult community homes throughout the city of San Antonio and one assisted living home on the main Mission Road campus for adults. Residents living in the homes are assisted by well-trained, 24-hour AWAKE staff. During the week, residents spend their day working, training for a job, volunteering or engaging in a social/habilitation program. Evenings are spent acquiring and refining the skills needed to live a more independent and fulfilled life. Weekends are for shopping, completing chores, recreational activities and spending time with friends. Apartments Mission Road Ministries sponsors a supervised apartment program for adults with intellectual and other developmental disabilities who need minimal supports. The goal of the program is to provide supportive services to optimize the independence and productivity of the adults in an affordable living arrangement. Mission Road has three apartment complexes in San Antonio that provide affordable housing and services for the tenants. Suzanne Smith Management Company supervises the three apartments: Independence Square, Murray Manor and Meadow Brook. The apartments are Housing and Urban Development (HUD) subsidized apartment complexes designed exclusively for adults with intellectual and other developmental disabilities. The apartments have one or two bedroom options with a community room for gatherings and a laundry room.

We will take the time to get to know you and develop an individualized care plan that fits your specific needs. Seniors 2000 adult day care program is designed to meet the needs of adults with functional impairments through individual plans of care by providing health, social, and related support services in a safe environment. This may include light exercises, individual floor dances,a pool table, table games, bingo, arts and crafts, and occasional "Zumba". Our centers have an open space setting for wheelchair bound individuals to move around. We provide a clean, safe, and comfortable environment for our clients to spend the day. Services include: • Transportation. We arrange for our clients to have safe transportation to and from their homes daily, making adult care services convenient. We hold Field Trips once in a while, enjoy visiting local libraries, local shops and or parks with our staff members accompanying a small group of clients. We keep our daily activities fresh everyday. Boredom is a thing of the past. • Therapeutic Activities. Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our adult day care can assist you with daily tasks such as mobility, eating, exercising, and grooming. • Healthy Meals. Individuals with developmental disabilities who do not have Medicaid funding can private pay for services in a 3-bed or 4-bed group home. We also serve individuals who live on their own in an apartment setting. RMI's Semi-Independent Apartment program gives adults with intellectual and developmental disabilities the ability to live in the community with more freedom and more responsibility. Residents in this program lease their own apartment and pay their own expenses. They may choose to live by themselves or share an apartment with a roommate. At San Mateo Apartment Homes, Waters at Elm Creek and other apartment communities in San Antonio, RMI residents work, play, shop and worship with others in their neighborhoods. With ongoing support from RMI staff, residents are able to meet the everyday challenges of living independently.
