Live Oak Community Health Center provides these senior care services:

• Acute care for geriatric patients
• Chronic disease management
• Preventative health care (physicals and annual exams)
• Diabetic foot exam
• Immunizations – Flu, Pneumonia, and Shingles
• PSA screen for male patients
