Our staff specializes in skilled nursing care, long-term care, and rehabilitation services as well as hospice and respite care. Our medical director and nursing staff provide a wide range of health care services for those with multiple needs and diagnoses including but not limited to:

Individual therapy involves a one-on-one, 45-50 minute, session with a trained therapist to examine personal issues that a client may be facing in a safe, supportive, and confidential environment. Helping Hands is a pre-vocational day habilitation program that serves higher-functioning adults with special needs who need a place to grow, develop skills, and establish life-long friendships. Helping Hands offers daily tasks and activities to adults who have intellectual and developmental challenges. Together, we serve our church, our community, and one another.

Helping Hands provides opportunities that bring purpose and dignity to youth and adults who have intellectual and developmental challenges. These opportunities encompass spiritual growth, education, fellowship, and service to God and our community.

Each day at Helping Hands is filled with service, fun, and fellowship! Our tasks are geared toward serving others and many of them promote independent living skills. In addition to our tasks and activities, we eat out at area restaurants and go bowling regularly. Physical activity is also a part of our day.

This program is for higher functioning adults with special needs.

LIDDA services include determining a person’s eligibility for services, enrolling a person into programs, and coordinating and monitoring on-going services for a person by assigning them a Service Coordinator. Medication-assisted treatment (MAT), is the most effective intervention to treat opioid use disorder. Combining the models of psychosocial therapy and the FDA-approved medication Suboxone, MAT significantly reduces illicit opioid use compared with nondrug approaches and increases access to therapies that can reduce overdose fatalities.

Meals on Wheels volunteers deliver hot meals, Monday through Friday to seniors, the homebound, and disabled in Alpine, Marathon, and closely surrounding areas, such as Sunny Glen. Volunteers also provide a chilled sack breakfast and lunch every Friday for clients who need food over the weekend.

PhyTEx athletic trainers work closely with area physicians, hospitals, coaches, and parents to ensure injury prevention and diagnosis, acute care, rehabilitation, and safe return to athletic activities.

PhyTEx accepts Medicaid and Medicare (pre-certification may be needed), Worker’s Compensation Board, military insurance and all major private insurance coverage.

Care Coordination staff assess the needs of the older individual or the family caregiver and effectively plan, arrange and coordinate needed services with the goal of maintaining independence or supporting the needs of the family caregiver. Through Care Coordination, we are able to purchase the following services on a client’s behalf from local vendors:

  • Home delivered and congregate meals
  • In-home services such as personal assistance services, homemaker services and respite care
  • Adult day activity services
  • Transportation
  • Minor home repairs or modifications 
  • Medical equipment, supplies, medications 
  • Assistance with the hearing aids, eyeglasses and dental services
