Care Locations

Our goal is to provide hospice services in the environment that best meets the needs of the individual patient, caregiver, and other family members. To that end, we provide care in whatever setting our patient calls home:

  • Personal Home – When possible and desired, we help our patients remain at home during their terminal illness. Whether care is provided in the patient’s home or a caregiver’s home, our team of professionals and volunteers makes sure that the appropriate equipment and supplies are placed in the residence so that it is as comfortable and well-equipped as possible.
  • Nursing, Assisted, and Independent Living Facilities – When caring for a patient who resides in a nursing home, an assisted living, or an independent living facility, our hospice team complements the services and skills of the facility staff and works with that staff to care for the patient. In addition, we offer our full spectrum of support services to the patient’s family.

The hospice care team that we bring to our  clients and their families includes:

  • Nurses
  • Nurse Practitioners
  • Social Workers
  • Therapists
  • Patient’s physician
  • Hospice medical director
  • Home health aides
  • Bereavement counselors
  • Chaplains
  • Volunteers

The families of children with multiple disabilities face unique, daily challenges. They seek knowledge about their child's disabilities and the impact of those disabilities on the child's abilities. They seek to identify realistic expectations for their child. They seek understanding, allies and emotional support.

TEAMability's monthly Parent Forums provide the opportunity for family members to share their knowledge and resources, learn from one another and experts in the field of disabilities, make friends, and become a united voice for their children. Family Centered Parties and Outings strengthen family relationships and are an opportunity for families to relax and have fun.

We serve parents, caregivers, and siblings of children 0-22 years who have combined multiple disabilities - cognitive, communication, sensory, motor and medical fragility.

No needles, chemotherapy, or surgery on this day. From the moment they’re picked up in a Ferrari, the excitement begins. They arrive at a celebrity destination of their choice (the spa, mall, games, dining, and Spurs events) and receive a V.I.P experience, complete with red carpet, ropes, paparazzi, and autograph-seeking fans. It’s a day they will never forget and allows them to have fun and simply be a kid.  It’s a beautiful day filled with fun, love, hope that they and their families can enjoy.
