The Community Services Case Management Program (CSCM) staff provides comprehensive case management for children and young adults (3-21) with intellectual or developmental disabilities or special healthcare needs, who are not currently covered by Medicaid.

Case managers work with families to identify needs, create individualized plans, and connect with resources.

• Diagnosis of an intellectual and/or developmental disability (IDD), autism spectrum, cerebral palsy, Down syndrome, dual diagnosis with IDD.
• Individuals of any age with cystic fibrosis.
• Ages 3-21 years.
• Must reside in Bexar and surrounding counties
