?Respite care is short-term relief for in-home hospice caregivers. Medicare benefits pay for patient transport and up to five consecutive days of inpatient care at a Medicare-approved nursing facility or hospital. You can get respite care more than once, but only on an occasional basis.

Hospice respite care enables the caregiver to rest and recover from the demands of daily in-home care. Respite care is also appropriate when a patient’s medical condition warrants a short-term inpatient stay for pain or other symptoms that cannot be managed at home.

Home-based therapy takes place in the client’s home rather than in the therapist’s office for people who would otherwise have a difficult time accessing therapy.

With financial burdens and conflicting schedules, we understand life gets a little unmanageable. Do not be discouraged. Our counselors are willing to make In-Home visits. You will receive the same experience as if coming into the clinic. St. Cuthbert is the patron for Our Lady of The Atonement's parish home visitation ministry for our parishioners who are not able to participate in parish life due to illness or other limitations that prevents them from leaving the house. We need volunteers who are willing to donate an hour a week to let our fellow parishioners know that we have not forgotten them.

We are an outreach ministry whose mission is to bring Jesus in the Eucharist to you in your home.  In an effort to keep our faith community connected, we minister to our parishioners who cannot attend Mass or receive the Eucharist due to illness or disability.

Our ministers visit once a week on the weekdays at a time that fits your schedule. We bring the presence of Jesus in the Eucharist, the blessings of our family of faith, and a prayer service. We package this into a visit from one of our friendly ministers to brighten your day. 

Requirements:

  • Recipients must reside within the parish boundaries. Call the Parish office to determine eligibility.
  • Requests must come from the person requesting the service or a family member.
  • Requests may also come from the chaplain or social worker of a licensed group home or other facility

Please be aware that most of our San Antonio hospitals have Catholic Chaplains and Eucharistic Ministers who bring Holy Communion to their patients.  Be sure to request these services when you are in the hospital.

I believe the miracle of birth is a normal natural process and every person has the  right to choose where and how they give birth. My goal is to assist you with your transition into new parenthood.

 

 Throughout the birth process you will receive labor support as needed, birthing person and fetal assessments will be completed, you will have freedom to move, eat and drink as you desire. Water birth can be accommodated while you enjoy the comfort of the warm water. Skin to skin bonding with your infant immediately after birth and breastfeeding/chestfeeding support without interruption is offered. Constant evaluation and observation of the birthing person and baby during the postpartum period for several hours is provided all the while allowing for family bonding. 

 

Postpartum visits are scheduled between 24-48 hours after the birth, an optional 5-7 day office visit, a 2 week  office visit for baby and birther and and a 6 week visit for the birther. During this period I will be available for any questions or concerns you may have. Certified Nurse Midwives can care for newborns up to 28 days of age, With this being said, it is encouraged you arrange a primary care provider for your baby as soon as possible. Often, people cannot attend Mass due to illness, surgery, or other incapacitation and thus cannot receive the Eucharist.

Our Homebound Ministry brings the Eucharist to them in their homes during these times. The duration of their need may be short-term or indefinite. The days and times for visits may vary according to need. They can be very flexible but are usually once per week and are arranged between the assigned minister and the parishioner in need.

The ministry is a wonderful opportunity to bring Jesus in the Eucharist to those who could not otherwise receive Him, to help them keep in touch with our Church Community, and to form special relationships. Training is available.

Agave Home Health Occupational Therapists evaluate and develop treatment plans to restore and improve a patient’s independence in performing activities of daily living, such as bathing, dressing, transfers, housekeeping, etc. The Occupational Therapists assist patients in improving and restoring lost and/or impaired functions, and to adapt to functional limitations affecting the activities of daily living.

After a home health episode or hospitalization, your physician can provide an order to continue therapy in your home. You do not have to be homebound to qualify.

The primary goal of therapists at Agave Home Health is to enhance the adaptability of patients to their home environment and to help them achieve the highest lifestyle functionality given the limitations of their medical or physical conditions.
